All Exit IEPs must be done by April 30 and turned in by May 10.

An exit IEP can be combined with the ACR for the rest of this school year.

  1. Purpose Tab:
    • select Exit IEP.
    • In the text box below, write:"Student is exiting High School with a Diploma/Certificate of Completion", or "Student has reached age 22 and is exiting Young Adult Services"
  2. Evaluation Tab-->Present Level SubTab
    • can be updated with information about credits earned and readiness to graduate. 
    • For YAS:  Either here or in SOP, give info about vocational experiences, work skills, communication skills, or anything else that would be helpful to adult service providers.
  3. Outcomes Tab: Testing Options- Note current status
    • If Exting and not passed GQE: Student will participate in GQE retake.
    • If Exting and passed ECA: Student has participated in state assessment prior to this year.
    • If Exting and not passed ECA: Student will participate in End of Course Assessment with/without accommodations.
    • If certificate track: Student is not on a diploma track, therefore iStar will be administered.

  4. Goal Tab: Add at least one goal based on future expectations:
    • For example:        
      • Student will contact Voc Rehab/Logan Center/BDDS
      • Student will contact college admissions office
    • State Standards (for example): Will participate in opportunities to learn…. (Find this in: Standards - Functional Achievement - Soc/Emo - Practices Habits of Learning - Participates in opportunities to learn
  5. Provisions Tab
    • Add only one Transition Service: Exit Survey to be completed by student and TOR. Date of completion is the last day of the school year.

    Exit IEP Services

    • Add only one Special Ed Service: Exit Consultation, with Initiation and Duration Dates of the date of the school year to the last day of the school year and 0 minutes. [IEP will not finalize without services]

  6. Placement Tab
    • Mark Option 50 for LRE [Software will not allow you to finalize without filling in these options
    • "Student is exiting High School with a Diploma/Certificate of Completion", or "Student has reached age 22 and is exiting Young Adult Services"
    • Course of Study: "Student is exiting High School with a Diploma/Certificate of Completion", or "Student has reached age 22 and is exiting Young Adult Services".
    • General Considerations: Leave options and statements from prior IEP.
    • Program Info: Leave options selected for prior IEP and add statement in Other Program Information Text Box: "Student is exiting High School with a Diploma/Certificate of Completion", or "Student has reached age 22 and is exiting Young Adult Services"
    • On the SOP, copy and paste all of the accommodations that are being given to the student from prior IEP. Be sure you remember to print the SOP separately from the File Cabinet after the IEP has been finalized.

  7. File Tab: When you finalize put the date of the exit IEP conference to the end of the school year as effective dates.
  8. Make sure you print out the SOP separately when you finalize the IEP.
